Police have identified the man who died after a shooting in Manchester on Monday.
Police said they responded to a home at 158 Eldridge St. around 1:25 p.m. and found the victim, later identified as 45-year-old Joseph Swan, of Manchester, with a gunshot wound to the head.
Swan was taken to Manchester Memorial Hospital, where he later died.
The incident is being investigated as a homicide at this time, police said, and investigators are actively following leads in the case.
